How to get your GP record

Your GP record includes information like any conditions or allergies you have and any medicine you're taking.

All patients are entitled to get a copy of their medical record under the General DataProtection Regulations (GDPR). 

Patients are asked to complete and return a Subject Access Request (SAR) and you will be provided with your notes within 30 days. There is usually no charge for this service.
